FIRST! Please use the Appointment Determination Process (ADP) in order to determine where you should go for tax assistance.
Why should you use the Appointment Determination Process?
Because there are many different types of tax situations possible, we have developed a process to tell tax payers which kind of service each person needs and who will prepare tax returns for them.
To satisfy that request, you must use the Appointment Determination Process (ADP), which asks you to provide certain information about yourself In return it generates a message that tells you what to do based on the information you provided. Read the information on that page and select the link to “appointment determination process”.
Print out the result of the Appointment Determination Process (ADP) for later reference. Bring the printout with you to the ISSA if you have been directed to make an appointment for an individual tax assistance session.
A tax return cannot be prepared without all the evidence of income forms (w-2, 1042-S, etc.). That means those with treaty benefits cannot make an appointment until after receiving Form 1042-S. If you should fail to receive a W-2 or 1042-S, you may need to request a replacement by submitting the following form to the Payroll office: http://controller.nd.edu/forms/ProceduresforDuplicateFormw-2Requests.pdf
You will be given an appointment confirmation and a four-page questionnaire which you must fill out before your scheduled individual session. See the following Web page for a list of documents that you should retain and bring with you to your tax appointment, in addition to the four-page questionnaire. YOU MUST HAVE ALL YOUR REQUIRED DOCUMENTS AND THE COMPLETED QUESTIONNAIRE WITH YOU FOR THE INDIVIDUAL SESSION. TAPtax will provide the tax return forms.
